Completion of the daily COVID screen form is no longer required at this time.
We ask however that if the individual is tested or tests positive for COVID that we are notified within 24 hours so a report can be entered per our regulation.

EVV is Live as of January 1, 2020
All SSPs must be checking in and out for their shifts using the CellTrak mobile app. Due to the new government mandate, paper timesheets will not be accepted for any hours worked from February 2, 2020 forward.
As of July 5, 2020, all CellTrak users must submit their Progress Notes electronically prior to logging off of the app. For IVR users, Progress Notes are to be completed weekly and submitted no later than 4pm each Tuesday to awc@yourawc.org.
Managing Employers can find the weekly scheduling form on the EVV resources page. Weekly schedules should be sent to schedule@yourawc.org for submission. They are now due the Tuesday before the schedule begins.
